Assistance Form for Rent & Utilities

The name and address must match the bill with no exceptions. Send an email to get_into_islam@yahoo.com. The form will be reviewed and if filled out correctly on to the assessment. 

The Assessments will be based on the information in the form, the attachments and whatever information is gained from the interview process. 

Funds will be raised by our group, however we do ask everyone who gets assistance to donate when they can. That is also part of the process. This is not ran by the government at all. We are Muslims  helping those in need.
